Terms of Service

These terms govern your use of depressionandanxietyrelief.com and the telehealth services provided by Haven Health & Wellness. By using this website or booking an appointment, you agree to these terms.

Effective date: September 2026

Not Emergency Care

Our services are not for emergencies. If you are experiencing a medical or mental health emergency, call 911 or go to your nearest emergency room. If you are in crisis, call or text 988 (the Suicide & Crisis Lifeline), available 24 hours a day, 7 days a week.

Eligibility

  • You must be at least 18 years old, or be accompanied by a parent or legal guardian who consents to treatment.
  • You must be physically located in a state where our clinicians are licensed at the time of your visit.
  • You must provide accurate, complete information when booking. Care based on false information may be unsafe and may result in termination of services.

Informed Consent for Telehealth

By booking and attending a video visit, you give your informed consent to receive care by telehealth. This means you understand and agree that:

  • Telehealth involves the use of electronic communications to enable a clinician at a different location to evaluate, diagnose, and treat you.
  • Telehealth has potential benefits, such as easier access to care, and potential risks, such as technical interruptions, limitations in performing a physical examination, and the possibility that your condition may require in-person care.
  • You have the right to ask questions about telehealth, to refuse telehealth services, and to withdraw your consent at any time without affecting your right to future care.
  • The confidentiality protections that apply to in-person visits also apply to telehealth visits, and no visit will be recorded without your consent.
  • You may request an in-person referral at any time, and your clinician may require one if telehealth is not appropriate for your condition.

ADA & Accessibility Policy

Haven Health & Wellness is committed to making our services accessible to everyone, including people with disabilities, in accordance with the Americans with Disabilities Act (ADA) and applicable state laws. We provide reasonable accommodations at no charge, which may include:

  • Accessible telehealth technology, including closed captioning on request.
  • Communication aids such as relay services, interpreters, or materials in alternative formats where reasonably available.
  • Website accessibility features, including text resizing, high-contrast mode, and reduced-motion settings available from the accessibility menu on our site.

To request an accommodation or report an accessibility barrier, contact us at info@depressionandanxietyrelief.com and we will work with you promptly.

Non-Discrimination Policy

Haven Health & Wellness does not discriminate on the basis of race, color, national origin, age, disability, sex, gender identity, sexual orientation, religion, marital status, or any other characteristic protected by federal or state law. We provide free aids and services to people with disabilities to communicate effectively with us, and free language assistance services to people whose primary language is not English where reasonably available.

If you believe you have been discriminated against, you may file a grievance with us at info@depressionandanxietyrelief.com, or with the U.S. Department of Health and Human Services Office for Civil Rights.

Nature of Telehealth Services

Our services are delivered by licensed clinicians via secure video visits. Telehealth has benefits and limitations: it does not replace all in-person care, technical issues can interrupt a visit, and your clinician may determine that your condition requires in-person evaluation and refer you accordingly. By booking, you consent to receiving care by telehealth.

Appointments & Booking

  • Appointments are requested through our online booking system and are conditional until your insurance information and required documents are verified.
  • Required documents — including a photo ID and insurance card — must be submitted at booking. Your appointment is confirmed only after verification is complete and approved.
  • Initial visits are approximately 45 minutes; follow-up visits are approximately 30 minutes.
  • You will receive an email once your appointment is approved and confirmed.

Deposits, Co-Pays & Insurance

  • A $50 deposit is collected before each visit. If your insurance approves coverage, the deposit is refunded or applied toward your co-pay, deductible, or coinsurance. If coverage is denied, the balance of the visit fee is due.
  • When a co-pay amount is collected upfront and your insurance later determines your responsibility is lower (including $0), the difference is refunded to your original payment method. Card refunds typically take 5–10 business days to appear.
  • You are responsible for knowing your plan's telehealth benefits. We verify benefits as a courtesy but cannot guarantee payment by your insurer.
  • Self-pay rates are listed on our pricing page and are due at the time of service.

Cancellations & No-Shows

  • Please cancel or reschedule at least 24 hours before your appointment by contacting us at info@depressionandanxietyrelief.com.
  • Late cancellations and missed appointments may result in forfeiture of your deposit.
  • If you are more than 10 minutes late to a visit, it may be treated as a no-show at the clinician's discretion.

Prescriptions

  • Prescriptions are issued only when clinically appropriate, at the sole professional judgment of your clinician.
  • Controlled substances may require an in-person evaluation under federal and state law and are never guaranteed.
  • We do not guarantee refills; follow-up visits may be required for continued medication management.

Patient Responsibilities

  • Attend visits from a private location with a reliable internet connection.
  • Provide honest and complete health information, including current medications and medical history.
  • Do not record visits without the written consent of your clinician.
  • Treat staff and clinicians with respect; abusive behavior may result in termination of services.
